Jason passed away unexpectedly on October 16, 2017. He was born on November 30, 1976 in Cedar Valley, Utah. He married his sweet Sara on November 30, 2016 in Oahu, Hawaii. Jason lived his life helping others and enjoyed spending time with his wife, children and family. He LOVED baseball…go Cubbies!!!
Jason is survived by his wife Sara, his sons Brayden Cook and Cayden Boothe, daughters Lexi Reynolds and Ambria Reynolds, mother Dee Ann Cook, father Melvin Cook, sisters Kim Miller (Neil), Michelle Wood (Matt) and Stephanie Lindsey (Brynt) and many nieces and nephews.
Funeral services will be held at 11:00 a.m. on Monday, October 23, 2017 at Larkin Sunset Gardens, 1950 East Dimple Dell Road (10600 South), Sandy, UT. A visitation will be held Sunday evening from 6-8 p.m. and Monday morning from 9:30-10:30 a.m. prior to the services.
